Procedure: Breast Implant Removal

43-year-old female who presents with a history of bilateral breast augmentation done in a submuscular plane using Mentor style 350, smooth, round, moderate profile, 375 cc saline implants (Ref#: 350-1660) in 2003.

For the past several years, starting about 1 year after her augmentation, she started feeling several nonspecific symptoms such as chronic pain, fatigue, hair loss (worsened over the past 5-6 years), joint pain, cognitive dysfunction, weight gain (of 75 lbs since her augmentation), headaches, poor sleep/insomnia, ear ringing, anxiety and drepression, numbness/tingling in the lower extremities, and a painful/burning sensation in her underarms. She had been diagnosed with hypothyroidism prior to her augmentation and had experienced some anxiety, hair loss, and fatigue, which worsened after implant placement.

More specifically, she has had pain in the L axillary area for a while and firmness in the bilateral upper poles. She also constantly feels swollen and chronically fatigued, which she has reported to her other doctors for several years. She thinks that her symptoms may be related to her breast implants and capsule, and has some concern about breast implant illness. She is interested in bilateral implant removal with capsulectomy, areolar reduction, and mastopexy.

She has absent nipple sensation at baseline. She did breastfeed her one child before her breast augmentation, and noticed a change in her breast size from an A to D cup with breastfeeding. She also had stretch marks on her breasts after breastfeeding. She has gained about 60 lbs over the past 4 years, and had weighed 140 lbs at the time of her augmentation.

Her bra cup size was a 34A before implant placement. After implant placement her bra cup size was a full C and she is currently a DDD. She would ideally like to be smaller, and would be okay with a B cup. She has never had a mammogram or any other imaging for her breasts.

She is shown 6 months after en bloc capsulectomies, bilateral breast implant removal, and mastopexy with chest wall liposuction (04/28/2021). She is doing well with no complaints. She has improved sensation in her nipples, and the pain in her breast has resolved. She was very happy with the result and improvement in all her constitutional symptoms.
