The mommy makeover has evolved over the years. It used to be thought of as just some form of tummy tuck and breast implants. However, it's really the combination of a tummy tuck and breast surgery, but it can encompass anything that gets patients back to a body shape and size that they feel comfortable with, whether that occurs after pregnancy or after weight loss.

Sometimes that means breast and body surgery, breast and arms, breast and thighs, or abdomen and thighs. It's really this idea of a combination of surgical techniques to restore the body shape.

Are patients still looking for a total transformation or a subtle restoration of their pre-pregnancy anatomy?

Most of the patients I see in Portland for a mommy makeover are looking for this pre-pregnancy restoration. Patients do want dramatic results—I love dramatic results—and it has to do with, in some cases, extra skin or things that have relaxed. One of the common things I see with patients in their post-pregnancy bodies is rectus diastasis, or a stretching of the abdominal wall. It's not quite as taut as it used to be.

Really, we are trying to restore that pre-pregnancy body shape in terms of tightening skin, removing extra skin and fat, tightening the abdominal wall, tightening arms, and lifting breasts. There's a lot of things you can do, and it really just depends on what patients want.

If they're looking for volume, in some cases, I see patients where they want a tummy tuck, but when we talk about a breast lift, they don't really want the extra volume associated with implants. So for them, my Portland mommy makeover is a tummy tuck with liposuction and a breast lift. It's totally tailored to what patients are looking for. That customized approach is important, and that's what I do: patients come in, we talk about what their goals are, and then we talk about how we can achieve that with their anatomy and what I recommend as a surgical plan.

How do you help a patient decide which combination of procedures will best achieve their specific goals?

When patients come to see me in Portland for a mommy makeover, we have a talk about their goals and personalize that to the areas that they're most interested in changing, whether it's the breast, abdomen, or their arms. It has to do with really looking at how much we can do in a given surgery. We have a finite amount of time, so we look at what procedures we can safely combine in a safe amount of surgery and what their major goals are.

I think it's really trying to tailor that particularly for their anatomy. In some cases, patients are candidates for more than one surgery, and we talk about the options, particularly whether it's the abdomen. We talk about balancing longer scars for a greater improvement in contour versus shorter scars, or a mini versus a full abdominoplasty.

Sometimes a more extensive surgery is going to give a more dramatic result, but sometimes patients don't want the downtime associated with something more dramatic. So we talk about options, tailoring it to what they want, their anatomy, and what I can deliver surgically so we can come up with a plan that makes them comfortable with how their body is going to be changed.

Can a tummy tuck during a mommy makeover fully repair diastasis recti (separated abdominal muscles), and how does that benefit a patient's daily physical comfort?

Diastasis recti, or rectus diastasis, after pregnancy is very common. It is a separation of the rectus abdominis muscles—the six-pack muscles—in the midline. It separates over time, and after pregnancy, sometimes those muscles will go back into their natural position. Sometimes physical therapy and some strengthening exercises can be helpful, but in some cases, that stretching is really dramatic, and patients almost have a big bulge, a loose abdominal wall, or it may even appear like they have a hernia.

The repair of that rectus diastasis can be done, and typically I do that as a part of all standard abdominoplasties. Not for a mini necessarily, but for a regular abdominoplasty or the more extended tummy tucks, that rectus diastasis gets repaired and tightened because you really need to have that abdominal wall tighter and flatter to get the best-looking result. If you don't, things still look kind of stretched out. You want a nice, flatter tummy, and this really helps to restore the natural shape and position of the rectus muscles. It's very important.

What safety protocols does your practice follow to ensure a safe and effective experience, especially when combining multiple procedures into one surgery?

Patient safety is paramount. We look closely at the overall amount of surgical time. In terms of safety things we do for surgery, first, it's perioperative antibiotics. We also use special devices on the legs to help prevent blood clots during surgery.

Additionally, we keep the total amount of surgery time down. Typically, I don't like surgeries to go over maybe six or seven hours tops, especially if you're doing combined procedures. The reason is that when you start to get over that amount of time, the risk of complications statistically goes up. Doing 12-hour marathon procedures, although you may read about this or see where some celebrities have had it done, is not something I recommend. I like to keep those times down and perform safe combination procedures.

In order to minimize the risk of blood clots for patients who may have a higher baseline risk, they will also get blood thinner medication provided around the time of surgery to add a little bit of a safety margin.

What is the recovery timeline for a busy parent?

For a mommy makeover, women who have little kids are generally still going to be six weeks out before they're able to resume full activity. That timeline has to do with the repair of that rectus diastasis muscle. Those sutures are holding that inner part of the abdominal wall together.

If you go back and do too much lifting and straining too soon, it's going to stretch things out and really compromise the result. Patients have to get a little extra help or arrange things to make childcare a bit easier so they're not lifting and potentially straining, because we want to protect that muscle while it's healing.

How soon after breastfeeding can I get a mommy makeover?

I generally recommend patients wait at least 6 months. The breasts are going to involute, meaning they get softer and shrink a bit after breastfeeding is completed. We really want to get all that milk out of the breast.

They will go down in terms of size and sometimes they'll become a little more ptotic, or stretchy. Once they settle, patients have a better idea of the actual tissue we need to work with. If we do a breast lift during active breastfeeding, there will still probably be some unpredictable stretching and laxity afterward. It's really about finding the optimal time to do it, so there is a little bit of a wait that has to occur.

Can I have more children after a mommy makeover?

Yes, you can. Life happens, and sometimes patients get pregnant after a tummy tuck or a mommy makeover. The abdominal wall can absolutely still carry a child. It's just going to stretch the abdominal wall back out.

Ideally, it is best to wait until a woman is finished having babies before we do the tummy repair. We're going to be fixing that diastasis, and it's nice not to have to do it all over again. But yes, it is entirely possible.

What is the difference between a mini and a full tummy tuck in a mommy makeover?

There are some variations, but generally, a mini tummy tuck is just going to involve taking a small crescent of skin from the lower abdominal wall, which can possibly be combined with some liposuction. In most cases, the diastasis—the separation of the muscles—is not repaired during a mini tummy tuck. That's the whole idea of a mini; we're taking just a little bit of skin for patients who don't have a lot. They just want a little nip and tuck, and the recovery is a bit easier with less downtime associated with it.

A standard or full abdominoplasty is going to be a longer surgery and will typically involve a longer scar. This addresses a larger excess of skin and fat, and it includes that repair of the rectus diastasis separation. Generally, there's going to be a neoumbilicus, meaning a new belly button created as well.

That apron of skin gets removed, and then the skin above the belly button is pulled down like a window shade before the new belly button is brought through. There is a bit more scarring that goes with a full abdominoplasty. There are also other variations like the fleur-de-lis, a belt lipectomy, or an extended abdominoplasty, which generally just use longer scars to address more skin, but follow the same core idea.
